My Transition Roadmap

The road to adult care is not a straight path. There are many ways to get there, and there may be some challenges along the way. Learn more about the steps involved in transitioning to adult care below.

1. Starting the Transition

Your healthcare team will talk with you about the idea of transition and what you need to do before you can move to an adult healthcare team.

2. Keeping Track of Information

Your healthcare team will talk with you about the idea of transition and what you need to do before you can move to an adult healthcare team.

  • Work with your healthcare team to find an adult gastroenterologist
  • Learn how to book your own medical visits
  • Find out where to access support programs and where to find information on your health insurance coverage

3. Preparing for Adult Care

When you switch to adult care, you will take on more responsibility for your own care. You can prepare for this responsibility by understanding the knowledge and skills you need to manage your disease and use health care services.

This quiz will help you identify what you already know and what you might like to know about managing your IBD.

  • Review your results to figure out what skills or knowledge you need to spend more time on
  • Save the results of the ‘Am I Ready?’ quiz to your device
  • Share your results with your healthcare team
  • Repeat the quiz before your next health visit to see the progress you have made

4. Sharing My Plan of Care

Your adult healthcare team will need a full record of your past and present medical information to help you continue to manage your IBD. This includes:

  • Tests you have completed
  • Medications you are on
  • Your past hospital visits/stays

Your current healthcare team will help you to make sure your plan of care is up to date.
  • Know about the medications you are taking and medical tests you need to do
  • Know who to talk to find information about your medications, and medical history (eg, medications, tests, hospitalizations)

5. Transferring My Care

Your current healthcare team will send a transfer package including your medical summary and relevant contact information to your adult healthcare team.
  • Review the transfer package with your current care team
  • Book a first appointment with your adult healthcare team

Keep track of your medical appointments with a calendar (either on your phone or in your agenda).

Transfer Completion

After you have transferred to an adult care team, your pediatric care team will follow up with your adult care team to confirm that you attended your first appointment.
