Inflammatory bowel disease (IBD) is a disease you will have for the rest of your life. When you are about 18-years of age, your care will transfer to an adult gastroenterology team who will continue to manage your care. In adult care, you will make choices about your health on your own and be expected to take charge of your health.

Healthcare transition means getting ready to transfer from your current (pediatric) care team to an adult care team. You can prepare for the transition by:
  • Building the skills and knowledge you will need to manage your health
  • Becoming comfortable managing your medications and appointments
  • Taking control and making choices about your health

It is important to prepare for the transition as you move to adult care so that you may have fewer:
  • Flares
  • Medication changes
  • Hospital admissions
